Vision Coverage is an Ancillary Benefit that is a separate coverage from Medical. Vision consists of the following components. They are the following:

 

Exam Copays: Usually, a fixed-dollar amount an insured is required to pay for services at the time of the visit.

 

Material Copays: Usually, a fixed-dollar amount an insured is required to pay for materials at the time of the visit.

 

Maximum amounts and product types they will pay for lenses, frames, and contact lenses.

 

Individuals: These products are not offered by individual carriers at this time.

 

Employer-Sponsored Coverage can be purchased through several carriers. This can be paid by the employer or can be offered on a voluntary basis (the employees pays the full premiums). Each carrier has specific participation rules but some carriers go as low as two enrolled employees.
